This is a simulation game in which players assume the role of a paramedic in the fictional city of San Pelicano. Players complete emergency shifts by answering calls, racing to scenes, talking to patients, and providing treatment before transporting them to the hospital. Several scenes depict victims fleeing from explosions, trapped under cars/debris, or suffering from various wounds. One sequence depicts police exchanging gunfire with suspects. Some emergency scenes depict bloodstains, blood pools, and spurting blood effects; corpses are sometimes seen with charred flesh or exposed entrails. One disaster area depicts a corpse with a crushed in/mutilated skull. During the course of the game, players can remove patients' clothing to treat ailments and wounds; female patients' breasts are depicted during these procedures. Players sometimes respond to emergencies involving drug overdoses; drugs such as ecstasy, MDMA, and amphetamine are referenced in the text, with syringes and pills depicted near patients. The word “sh*t” appears in the game.
